Pre-authorisation is required for:

• Hospitalisation (elective and emergency)
• Cancer treatment
• Renal dialysis
• Specialised radiology
• Emergency evacuation
• Chronic medicines
• Appliance or prosthesis
• Organ transplants
• Ambulance services
• Specialised dentistry

Email or call us at least 48 hours before planned admission. For emergencies, notify within 48 working hours. Required info:

• Patient name, policy no., DOB
• Provider name & practice no.
• Hospital name & practice no.
• Reason & dates for admission/procedure
• Out-of-hospital procedures need provider details

Submit pre-authorisation forms via Forms and Documents. You'll receive a pre-authorisation number within 48 hours.

Claims must be submitted by you or your provider via email or post:

• Post: Liberty Life Assurance Uganda, Madhvani Building, Plot 99-101, Buganda Rd, Kampala

Ensure claims include: policy number, patient info, diagnosis, service dates, detailed treatment, tariff code (if any), charges, provider/facility details, total amount, pre-auth number (if any), and proof of payment for refunds. Claims are processed weekly and paid per benefit rules.

Register with the Chronic Disease Programme using the Chronic Medicine Application Form. Approved chronic meds are covered at 100% if within policy terms. Benefits also cover pathology, consults, and radiology. Notify us about any treatment changes for continued benefit use.
